                     Instructions on How to REGISTER your child for EVERY individual tournament

                           

                                           1.  Enter into our website at www.rgvjrgolf.org.

                                           2.  Click "2023 Schedule and Registration"

                                           3. Find the tournament you wish to register for and click "Register Now"

                                           4. Then fill out the type and quantity of Ticket you wish to purchase for.  (Age groups are provided for your assistance)

                                           5.  Fill out information about each golfer

                                           6. Checkout and pay through our new system with PayPal and our web site.

                                           7. Completed

                          LIABILITY RELEASE FORM

BY SUBMITTING AN ENTRY TO ANY RGV JUNIOR GOLF ASSOCIATION ACTIVITY,

YOU AGREE WITH THIS LIABILITY RELEASE FORM

 

In connection with the activities of my child as a junior golf participant in any tournament, clinic, seminar or instruction (the “Event”) provided by the RGV Junior Golf Association (the “RGVJGA”), I agree and understand that the presence and participation of my child in any event sponsored by the RGVJGA, its board members, membership, respective affiliates, agents (collectively referred to as “the Indemnified Parties”) may expose my child to both known and unanticipated risks of harm or injury.

 

In consideration of and as a prerequisite to the participation of my child, I acknowledge that such risks exist and I Hereby Assume All Such Risks, and Release and Discharge the Indemnified Parties and each of them and their board members, membership, respective affiliates, agents and any other participant from any and all claims of liability for personal injury or property damage my child may suffer while participating, including, without limitation, any claim arising out of any condition of any premises used by the RGVJA for Events or the conduct of any person, including other participants, in connection with the preparation for, supervision of or conduct of the Event or activity connected or related to it.

 

I Specifically Hereby Release the Indemnified Parties and each of them and their board members, membership, respective affiliates, agents and any other participant from any negligence of the Indemnified Parties and each of them and their board members, membership, respective affiliates, agents.

 

I Further Understand that the Indemnified Parties will not maintain insurance which will cover my child for personal injury, property damage or medical expenses, and I accept full responsibility for the costs of treatment for any injury or damages suffered while participating as a participant in connection with the Event.

 

I am fully aware that I must be at least 18 years of age to submit an entry without my parents’ permission.

 

If you are under 18 years of age your parents’ must submit an entry for you.

 

I have fully informed myself of the contents of this release by reading it before submitting an entry and agree to be bound by the terms set forth herein. 

                                                   

A $5.00 "NO SHOW" FEE WILL BE APPLIED TO YOUR NEXT ENTRY IF YOU DO NOT NOTIFY THE TOURNAMENT TO CANCEL FOR AN EVENT
